Indications:
Tofaxen (Tofacitinib) is indicated for the treatment of:

Rheumatoid arthritis (RA) – moderate to severe, in adults who have had inadequate response to methotrexate

Psoriatic arthritis (PsA) – in adults with inadequate response to nonbiologic DMARDs
-
Ulcerative colitis (UC) – moderate to severe active cases in adults

Juvenile idiopathic arthritis (JIA) – in pediatric patients aged 2 years and older (dose adjusted)
Pharmacology:
Tofacitinib is a Janus kinase (JAK) inhibitor.
It works by blocking JAK1 and JAK3 enzymes involved in the inflammatory process, thereby reducing the activity of immune cells responsible for chronic inflammation and autoimmune disorders.
Dosage & Administration:

Usual adult dose: 5 mg twice daily (or 11 mg once daily in XR formulations for some indications)

Taken with or without food

Dose adjustment may be needed in renal/hepatic impairment or with CYP3A4 inhibitors
Drug Interactions:

Use caution with CYP3A4 inhibitors/inducers

Avoid use with other potent immunosuppressants (e.g., biologics or cyclosporine)
