Indications:

Selinex (Selinexor) is indicated for the treatment of:

• Relapsed or refractory multiple myeloma (RRMM) in combination with dexamethasone in patients who have received at least four prior therapies.

• Diffuse large B-cell lymphoma (DLBCL), not otherwise specified, including DLBCL arising from follicular lymphoma, after at least two lines of systemic therapy.

Pharmacology:

Selinexor is a Selective Inhibitor of Nuclear Export (SINE).
It blocks exportin 1 (XPO1), a protein responsible for transporting tumor suppressor proteins out of the cell nucleus.
By inhibiting XPO1, Selinexor retains tumor suppressors inside the nucleus, reactivating cancer cell death pathways and suppressing tumor growth.

Dosage & Administration:

• Typical dose: 80–100 mg orally once or twice weekly, depending on indication and combination regimen.

• Selinex 20 mg tablets allow flexible dose adjustments.

• Must be taken with supportive care (e.g., dexamethasone, hydration, antiemetics).

Drug Interactions:

• May interact with CYP3A4 substrates and inhibitors.

• Use caution with medications affecting blood counts or causing nausea.

Contraindications:

• Hypersensitivity to Selinexor or its excipients.

• Use with caution in patients with severe nausea, vomiting, or frailty.

Side Effects:
